Apple phones you can use your camera to scan the QR code and a yellow link should pop on your screen.  

Android you can either download a free QR code app or go to naftalistatus.com COVID-19 testing site to get the link there.

There is two forms to fill out the first one is a quick consent form which is basic info the second one is the more important one that Has to be filled out per person who is taking a test.

You must have a / dash in the date of birth section or it will not go through

When it says browse files it is asking you to either take a picture or find in your files a picture of your insurance card [if you have] both sides and your ID only one side needed you need a ID to do this free Covid testing insurance

You should get results in about 2 to 3 days by email [we are really backed up now]

We are also offering personal rapid Covid testing which is $50 that we cannot yet give you a proper documentation test results you can take the test with you after

 I if you are filling out a form for more than one person or your child it is best to have more than one email because then the results that will be sent to you by email might get confusing so we highly recommend you use different emails because the system might think that you already did it already so please use separate emails per person or per kid  send me a copy of the responses may not always work so if you have completed the form and it’s still not working then just take off send me a copy of the responses

 if it says if you have no symptoms or if you see in the checkboxes if the answer is none of the above then you must fill out none of the above or the submission button or won’t work
